This is like learning all over again. Here's something I THINK I've discovered. Rearing a new level 9 priest with my new level 20 warlock, we sat out to work the "Hidden Enemies" progression. We clear the caves for an hour and nothing. I ungroup them, have the priest tag the mob then used the warlock to finish it. 2nd mob has the insignia for the cleric. The warlock still needed it so I left them ungrouped "just to see". Took 3 or 4 more mobs but it dropped for the warlock. My best guess is that a mismatched group size somehow messes up their quest drop algorithm.

For the books in Ragefire Chasm. We've not had the book problem (we've run it about 10x for the littler guys). Everybody got the books 1st or 2nd run through BUT, the book "Incantations from the Nether" has dropped 3x in a row from the same clump of mobs, those just beyond and to the left of Jergosh (normally the last boss). We'd kill those because our little toons were so little they couldn't loot Jergosh without pulling that clump of trash.

Again not specifically to THIS book (we're not sure which), my gf, brother and I confirmed between us that we got one of the books off the warlocks up the ramp before Bazzalan. This section might often be skipped because the Bazzalan is a quest only boss with no gear drops.

I don't think the places above are the only places that drop the books but there seems at least a good statistical chance to get them beyond Jergosh and before Bazzalan.
